National Championship Forms 
Team Rosters:
Every team competing at a CHEERSPORT event is required to submit the following:
- A team roster for each team.
These forms should be mailed to the CHEERSPORT office at 11010 Monroe Road, Suite B, Matthews, NC 28105.
